Patient Referral
In order to submit a new patient referral to the Regency Lodge, please send us the following documentation and information:
1. Referral Form
Download, print and complete a Patient Referral Form.
2. Covering Letter
A coverting letter requesting the patient be considered for assessment.
3. Patient Reports
Recent psychiatric and nursing reports and details of the patient's background.
Post all the above documentation to:
